38-year old man
Clinical Characteristics
Presented with near-syncope secondary to atrial flutter. Direct current cardioversion was successfully performed. After 5 months, the atrial flutter recurred accompanied by a cough, chest pain, fever, weight loss, and an elevated c-reactive protein. X-ray showed an enlarged right heart border. Us showed an echo-lucent halo near the right atrium and mild pericardial effusion. CT revealed a 10cm, hypodense cystic lesion adjacent to the pericardial surface of the right ventricle and atrium.both pericardium and wall of the cyst were thickened with contrast enhancement. Imaging studies suggested a symptomatic inflamed pericardial cyst. Empirical treatment with high dose aspirin/colchicine was initiated.
Remission Characteristics
Prior to diagnostic puncture, cardiac MRI was obtained which showed resolution of the lesion
