Case Details

Disease Location

Ileum

Personal Characteristics

78-year-old female. History of right femoral neck fracture operation, aspiration pneumonia, hypertensive cardiovascular disease, and diabetes mellitus

Clinical Characteristics

Presented with episodes of vomiting for one day. She was bedridden and totally dependent on her daily activities. Laboratory tests showed leukocytosis. Due to repeated vomiting, a nasogastric (ng) tube was inserted for decompression. CT scan delineated a 1.5 cm stone located at the terminal ileum, accompanied by distended proximal small bowel, collapsed distal small bowel, air in the gall bladder and common hepatic duct, suggesting the diagnosis of gallstone ileus. Diagnostic laparoscopy was performed.

Remission Characteristics

No apparent gallstone with obstruction was detected.

Treatment & Mechanisms

Proposed Remission Mechanisms

The stone might have passed through the ileocecal valve spontaneously

Clinical Treatment

Nasogastric tube, diagnostic laparoscopy.
