Personal Characteristics

20- months-old male

Clinical Characteristics

Since age of 14 days, he had been admitted due to several instances of dyspnea & wheezing found to have marked cardiomegaly & failed treatment of digitalis blood pressure 100/60 mmhg with frequent premature beats heart enlarged (grade 3/6 ejection systolic murmur heard over base & 2nd heart sound at pulmonic area widely split) electrocardiogram revealed sinus tachycardia with frequent ventricular premature beats & right bundle branch block chest x-ray film revealed marked cardiomegaly & large mass lesion on right side of heart at base angiocardiogram revealed large tissue mass on anterior right side of heart & compression of right pulmonary artery biopsy specimen consisted of fragments 0.8 x 1.2 cm in aggregate tumor composed of spindle cells entirely

Remission Characteristics

In ensuing postoperative years, dyspnea & wheezing improved gradually blood pressure, pulse rate normal heart not enlarged, no murmur heard chest x-ray film showed no evidence of cardiomegaly & pleuropericardial scarring on right mediastinal area present cardiac catheterization revealed no evidence of intracardiac shunts follow-up chest x-rays revealed no cardiomegaly, ecg unchanged & echocardiogram normal

Clinical Treatment

Right heart catheterization performed & revealed no evidence of intracardiac shunt exploratory thoracotomy performed year later--> large, multinodular mass arising from right ventricular wall extending to pulomary artery
