Abstract

Two cases of Hodgkin's disease are described who relapsed 22 and 19 years after regional radiation therapy. One of the patients also developed a carcinoma of the large bowel soon after first relapse. The importance of long follow‐up and documentation of cause of death in Hodgkin's disease is stressed.

42 -year-old male

Clinical Characteristics

Presented in 1962 with a 6 months history of painless left supraclavicular lymphadenopathy he was given 12 fractions of rad to that area following a biopsy diagnosis of hodgkin's disease. In march 1981, he presented with an 18 months history of weakness, shivering, night sweats, and 12.5kg weight loss investigations led to an abdominal ultrasound andat laparotomy in april 1981, a mass of nodes around the aorta and coeliac axis was biopsied the spleen was twice normal size histology of the node mass showed mixed cellularity hodgkin's disease, confirming relapse outwith the previously treated area sections from the origianl biopsy in 1962 showed identical features. Liver biopsy showed only a portal inflammatory infiltrate. He was staged at iib chemo was given in june 1982 relapse occured at 19 years later

Remission Characteristics

After the radiation, he remained well until march 1981 his relapse is in complete remission following chemotherapy in june 1982 with chlorambucil, vinblastine, procabazine and prenisolone
