Case Details

Disease Location

Left shoulder

Personal Characteristics

20-year old male; feel down the stairs

Clinical Characteristics

Anterior dislocation of left shoulder; 1x2cm bony fragment found in front of glenoid using radiographs; hill-sachs lesion noted

Remission Characteristics

CT scan taken 3 months after surgery showed a bony bankart incorporated into the original anteroinferior glenoid and resolution of the hill-sachs lesion; defect was filled with new bone; patient reported no pain and showed full range of motion 1 year after surgery

Treatment & Mechanisms

Proposed Remission Mechanisms

Immediate repair of bankart lesion and very early restoration of shoulder stability eliminated problems caused by unstable joint, which is thought to contribute to repeated injuries and non-healing lesions; relatively small size of lesion

Clinical Treatment

Arthroscopic bony bankart repair with 3 absorbable suture anchors; immobilization maintained with abdominaluction brace for 6 weeks; shoulder shrugging, active elbow flexion/extension, active forearm supination/pronation and active hand/wrist motion encouraged immediately after surgery; muscle strengthening initiated at 9-12 weeks after surgery
